HELPFUL INFORMATION
PRESCRIPTION FOR TREATMENT
Most patients come to us with a prescription for physical therapy that is issued by their physician and includes recommendations for treatment. Some insurance plans will not pay for physical therapy without a written PT referral, others will. If you have any questions regarding your specific insurance requirements, please contact your insurance provider.
PAYMENT INFORMATION
We are happy to verify your insurance benefits and our participation in your specific insurance policy prior to your first appointment. Please have your insurance card ready when you call to make your appointment. This will allow us to have your benefits confirmed before you arrive at your first appointment. Our cash rate is: Evaluation $125, treatment $100.
WHAT TO WEAR
We recommend that patients wear loose-fitting attire or exercise clothing and sneakers for maximum comfort.

Medication List
Reports from any diagnostic tests (X-rays, MRI, EMG, etc.)
Operative reports
Any braces, prosthesis, or assistive devices you are using

WHERE TO PARK:
LA CAÑADA: Aspire Physical Therapy and Sports Performance offers free parking under the office building or street parking. We are located upstairs on the third floor. For your convenience, there is an elevator in the middle of the Plaza, as well as several staircases leading to the third floor.
GLENDALE: You are free to park in any of the spots within the Elevate Fitness Complex.
PATIENT REGISTRATION FORMS
Please arrive 10-15 minutes early to allow time to complete your new patient registration forms, or take a few moments now to review and print the forms. When you arrive at our facility, our staff will greet you and review the forms. We will schedule all of your appointments and collect any payments (if applicable) at each visit.
MEETING YOUR THERAPIST
You will meet with a Doctor of Physical Therapy (DPT) who will evaluate your injury or condition, discuss your goals and specific needs and develop an individualized treatment plan.
NEXT STEPS
The length of your treatment plan and frequency of visits is based on your specific injury/condition, the physician’s prescription, your insurance plan, and the DPT’s recommended treatment plan.
HOME EXERCISE PROGRAMS
You may be provided with a home exercise program that compliments your plan of care. The home program is an important part of the healing process and will assist you in achieving your goals.
